Who to Refer?
Hypertension
· New diagnosis of hypertension
· Suspected secondary hypertension
· Resistant hypertension (≥3 medications)
· Difficult medication tolerance or side effects
· Routine screening for Primary Aldosteronism (systematic, guideline-based approach)
Obesity
· BMI greater 30 or greater than 27 with comorbidities
· Difficulty initiating or optimizing GLP-1 therapy
· Weight plateau or regain
Lipid Disorders & Cardiovascular Risk
· Elevated Lipoprotein(a) or strong family history
· Suspected or confirmed familial hypercholesterolemia (FH)
· Statin intolerance or complex lipid management
· Premature ASCVD or high-risk primary prevention
Diabetes
· New diagnosis of diabetes
· Complicated control or difficulty in reaching target A1c
· Type 2 Diabetes +/- additional cardiovascular risk factors
· Optimization of GLP-1 and SGLT2 therapies
Cardiology Consultation
· Chest pain assessment
· Palpitations & arrhythmia assessment
· Initial assessment and coordination of cardiac investigations (e.g. Holter, Stress Testing, Echo, Cardiac CT)
Liver Disease
· New diagnosis of hepatic steatosis
· Lifestyle and medical management of MASLD

· Timely access to patient consultation (urgent cases accommodated sooner)
· Clear communication and ongoing collaboration with referring physician
· Appropriate triage of cardiac symptoms to reduce unnecessary ER visits
